Non-urgent advice: Please note

Full payment is required prior to the completion of the report/documents or on booking of appointment for medical examination.  A 50% charge will be incurred if a patient fails to attend a pre-booked appointment or if the appointment is not cancelled with 24 hours’ notice.  The Practice does not undertake certification of identification documents, for example, driving licence and passport.

This list is not exhaustive. Other services may be available on request. Please contact the Practice Manager for details.  Under their NHS contract, GPs are not required to provide reports or offer an opinion on incapacity for work to anyone else other than Jobcentre Plus or DWP who will write to the GP direct, nor are we able to complete any “fit to” certificates e.g. fit to fly, participate in extreme sports etc. Unfortunately we are also not able to provide letters to support housing moves, school exemptions e.g. toilet passes. The NHS App has access to your medical record and you can share information from there with any third party .

Guidance for medical practitioners about common fees, including what and when to charge (bma.org.uk)

With certain limited exceptions, for example a GP confirming that one of their patients is not fit for jury service, GPs do not have to carry out non-NHS work on behalf of their patients.

Time spent completing forms and preparing reports takes the GP away from the medical care of his or her patients.  Most GPs have a very heavy workload and paperwork takes up an increasing amount of their time.

Non-NHS work is generally done in the doctor’s own time.

When a doctor signs a certificate or completes a report, it is a condition of remaining on the Medical Register that they only sign what they know to be true.  In order to complete even the simplest of forms, therefore, the doctor might have to check the patient’s entire medical record. Carelessness or an inaccurate report can have serious consequences for the doctor with the General Medical Council (the doctors’ regulatory body) or even the Police.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you will need to complete the travel health questionnaire either using the link below or by collecting a paper copy from the practice.  Ideally we need 12 weeks notice to ensure the correct vaccinations can be given.

There is further information about countries and vaccinations required on the links below

It is important to complete the questionnaire as early as possible – at least 12 weeks before you travel – as an appointment will be required with the practice nurse to actually receive the vaccinations. Some vaccines require a second appointment which ideally needs to be at least 2 weeks before you travel to allow the vaccines to work.

The practice is not able to offer some of the recommended vaccinations and you would have to source these at a private travel clinic; this is because not all travel vaccinations are included in the services provided by the NHS.
